Medical Case Management

We will always remain the number one advocate for our patients and are proud to offer collaborative medical case management to protect them. We work with our patients to facilitate recommended treatment plans and ensure that appropriate medical care is provided at all times.

As a lot of military and vets would attest, accessing many resources for assistance can oftentimes be complicated and incite certain levels of anxiety. We at SMC, have a tremendous amount of respect for the men and women in uniform, and as such, have implemented education and training, services and collaboration with the community, and national programs in order to serve you better.

We do not represent VAC, nor do we know all the details of their policies. We do, however, have an exceptional, ever growing amount of organizational relationships that bring much needed resources and alternatives in dealing with the unique trials and tribulations military/veterans and their families deal with for years on end.

Your access to healthcare benefits and services will depend on a number of factors, including your eligibility, health needs and individual circumstances.

You’re not alone, we are here to assist and support!

  • Chronic pain is an abnormal response and does not improve with time. It can occur with tissue damage and persist long after the body heals. It changes how nerves and the brain process pain, as misfiring nerve signals continue to tell the body it hurts.

We offer Chronic Pain Management advice to patients.

Lincoln Taylor MN NP CGPain, has joined our team and will be offering care and advice to patients who continue to suffer from Chronic Pain despite their improvement efforts.

Lincoln works thoroughly with patients to examine every aspect of their daily lives to determine where patients can make changes, in hopes to secure a better outcome with their treatment plans.

  • Massage therapy is the manual manipulation of the skin, muscles and skeletal structures of the body. The stretching, compression and rhythmic movement allow the body to reduce tension and improve overall wellness. Massage therapy can be helpful with headaches, migraines, joint and muscle pain, pre and post-op treatment, improving mobility and general overall health.

Cupping: A Traditional Chinese Medicine Technique that provides a deep tissue massage, dating back 1550 BC. It can provide a reduction in fascial adhesions and inflammation, increased circulation, relaxation, and improved mobility. Medical grade cups are placed on intended areas through mechanical suction, left to rest or with passive stretch. All cupping sessions are preceded and followed by massage therapy.

Hot Jade Stone: Not your typical hot stone, we’re talking Jade Stone! Known as the protective stone for any crystal lovers, the jade stone also retains heat/cold tremendously well. The stones are heated to 120°F/49°C, oils are applied to the skin, strategically chosen stones glide over with, as well as across, muscle fibres. This enables the therapist to attain a deeper tissue release without deep tissue discomfort.

  • Celluma light therapy uses unique technology based on NASA research to improve cellular health to reduce the signs of ageing, relieve pain and eliminate acne.

Unlike any other low-light therapy device available, Celluma delivers blue, red and near-infrared light energy simultaneously to safely treat a wide variety of conditions.

Clinical benefits include pain relief in conditions such as carpal tunnel syndrome, bursitis, tendinitis, ankle sprain, neck and shoulder pain, muscle tensions and many others.
